Questionnaire FAQs

How to license a questionnaire, deal with translations and other restrictions

What do I do if I want to license a questionnaire from McMaster?

Contact MILO's Intellectual Property Assistant. Please be prepared to describe which questionnaire you wish to use and your proposed use. Please include the following details:

  • the name(s) of the Principal Investigator(s)
  • full name and mailing address of your institution
  • title of your study
  • number of patients to be administered the questionnaire
  • number of study sites (if applicable)
  • if the study is sponsored
  • language versions required

Do I need to get a licence if I only want to use a questionnaire for academic research or non-profit purposes?

Yes, you must sign a licence agreement for any use of the questionnaires, whether it is commercial or academic. The questionnaires are protected by copyright and are the exclusive property of McMaster University.

Do I have to pay a licence fee?

Yes, a licence fee is payable based on the proposed use. The fee will depend on factors such as whether the use is for a clinical trial or academic study and how many subjects are involved. Contact MILO's Intellectual Property Assistant for more information.

Do you have translations of the questionnaires? If not, what do I do if I need to use the questionnaire in another language?

Translations are available for many of the questionnaires and are provided at no additional cost to the licensee. If you require a language that is not available, you must request permission to prepare the translation in writing and provide the name of the translation company. We will provide you with our Protocol and Pre-test information. You will work with an author of the instrument to complete the translation to ensure the integrity of the original instrument is maintained. A pre-test of five to 10 patients must be completed and the data submitted to McMaster for review. Upon acceptance of the translation, it may be used in your study and the data provided to McMaster for validation purposes only. All translations remain the property of McMaster University.

Can I modify the questionnaire to suit my purposes?

No, the questionnaire may not be modified. All questionnaires are protected by copyright. Any change made without the prior written consent of McMaster is a breach of copyright.
